Job summary

Acacia Health is seeking an experienced hospice nurse to plan, coordinate, and deliver home-based care. You will assess patients, develop and revise care plans, and work closely with physicians to meet patient and family needs in home and community settings.

Responsibilities include documenting in HomecareHomebase, educating patients and families, and serving as a case manager to ensure seamless care delivery across interdisciplinary teams.

Qualifications

CA RN License required.
CPR certification required.
CA Driver License required.
Hospice nursing experience: 1 year (Required).

Responsibilities

Completes an initial, comprehensive and ongoing assessment of patients.
Initiates the plan of care and revisions as needs change.
Develops a care plan based on nursing diagnosis and palliative actions.
Assists patient with activities of daily living and supports self-sufficiency.
Acts as Case Manager to coordinate patient care.
Completes and submits clinical notes in HomecareHomebase.
Communicates with physician regarding patient needs and changes in condition.
Teaches self-care techniques to patient and family/caregiver.
Attends interdisciplinary meetings.
Fulfills assigned case work.

Job Description Summary

The RN plans, organizes and directs hospice, and plans and directs services to meet the needs of patients and their families within their homes and communities.

Essential Job Functions/Responsibilities

Completes an initial, comprehensive and ongoing assessment of patients.
Initiates the plan of care and makes necessary revisions as needs change.
Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
Assists the patient with the activities of daily living and facilitates the patient’s efforts toward selfsufficiency and optional comfort care.
Acts as Case Manager and assumes responsibility to coordinate patient care.
Completes, maintains and submits accurate and relevant clinical notes in HomecareHomebase (Pointcare).
Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
Teaches the patient and family/caregiver selfcare techniques as appropriate.
Attends interdisciplinary group meetings.
Fulfills the obligation of requested and/or accepted case assignments.
