Job summary

Acacia Health (Palliative, Home Health, & Hospice) in California is seeking an experienced RN to join our hospice team serving Victorville and the surrounding communities. You will provide compassionate, patient-centered care on the road, coordinating services in the home and community setting.

The Registered Nurse will assess needs, develop and revise care plans with families, and collaborate with interdisciplinary teams to deliver high-quality palliative care in patients' homes.

Qualifications

• CA RN license required.
• CPR/BLS certification required.
• CA Driver License required.
• Hospice: 1 year experience preferred.
• Compassionate and caring.
• HomecareHomebase/Pointcare experience preferred.

Responsibilities

• Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
• Assists the patient with activities of daily living and facilitates the patient’s efforts toward selfsufficiency and optional comfort care.
• Acts as Case Manager and coordinates patient care for assigned caseload.
• Completes, maintains and submits accurate and relevant clinical notes in HomecareHomebase (Pointcare).
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Teaches the patient and family/caregiver selfcare techniques as appropriate.
• Attends interdisciplinary group meetings.
• Fulfills the obligation of requested and/or accepted case assignments.
