Job summary

Beth Israel Lahey Health in Exeter, NH is seeking a registered nurse to plan, organize, and direct home care services with emphasis on community health education. You will build from community resources to meet patients’ needs in their homes and communities in line with home care regulations and payer guidelines.

Key responsibilities include initial assessments, care planning, administering medications, supervising home health aides biennially, discharge planning, HIPAA compliance, OASIS

Qualifications

• Initial assessment of patient and family to determine home care needs.
• Plan of care and revisions as patient status changes.
• Administer medications and treatments as prescribed by physician.
• Instruct, supervise, and evaluate home health aide care every two weeks.
• Identify discharge planning needs as part of care planning.
• Maintain confidentiality per HIPAA and agency policies.
• Demonstrate proficiency in OASIS completion.
• Document in EPIC per agency timeliness rules.

Responsibilities

• Provide clinical oversight and direct home health services in patient homes.
• Collaborate with care team to ensure quality patient outcomes.
• Educate patients and families on health maintenance and safety.
• Ensure compliance with payer guidelines and home care regulations.
