Job summary

Beth Israel Lahey Health is seeking a registered nurse to plan, organize and direct home care services in patients' homes and communities with an emphasis on community health education.

The role involves initial assessments, care planning adjustments, medication administration, supervision of home health aides, discharge planning and HIPAA compliance, plus OASIS completion and EPIC documentation.

Qualifications

• Massachusetts RN license (active)
• Active BLS certification
• At least 1 year of direct care nursing experience (home health/public health preferred)
• OASIS and/or EPIC experience preferred

Responsibilities

• Complete and document an initial assessment of patient and family to determine home care needs
• Initiates the plan of care and revises as patient status changes
• Administers medications and treatments as prescribed by the physician
• Instruct, supervise, and evaluate home health aide care every two weeks
• Identify discharge planning needs as part of care plan development
• Maintain confidentiality per HIPAA and agency policies
• Demonstrate proficiency in OASIS completion
• Document via EPIC in a timely manner
