Qualifications

• Active Massachusetts RN license.
• Active BLS certification.
• Associate’s Degree in Nursing or BSN preferred.
• At least 1 year of direct care nursing experience, preferably home care/public health.
• OASIS and/or EPIC experience preferred.

Responsabilités

• Complete and document initial patient and family assessment to determine home care needs.
• Initiate and revise care plans as patient status changes.
• Administer medications and treatments as prescribed by the physician.
• Instruct and supervise home health aides every two weeks.
• Identify discharge planning needs within the care plan.
• Maintain confidentiality under HIPAA and agency policies.
• Demonstrate proficiency in OASIS completion.
• Document care via EPIC in a timely manner.
