Job summary

Beth Israel Lahey Health at Home in Woburn, MA is seeking a Registered Nurse to plan, organize, and direct home care services with a strong emphasis on community health education.

The RN will complete initial assessments, initiate and revise the plan of care, administer meds, supervise home health aides, and ensure HIPAA compliance, OASIS and EPIC documentation as part of patient-centered care. This is a full-time Day position with competitive pay and benefits.

Qualifications

• Active Massachusetts RN license.
• Active American Heart Association BLS certification.
• 1 year of direct care nursing experience, preferably home care/public health.
• OASIS and EPIC experience preferred.

Responsibilities

• Complete and document initial patient/family assessment to determine home care needs.
• Initiate and revise the plan of care as patient status changes.
• Administer medications and treatments per physician orders.
• Instruct, supervise, and evaluate home health aide care every two weeks.
• Identify discharge planning needs as part of the care plan.
• Maintain HIPAA confidentiality.
