Job summary

Beth Israel Lahey Health at Home is seeking a Registered Nurse for home health. The RN will plan, organize, and direct home care services with emphasis on community health education. You will assess patients, develop and adjust plans of care, and supervise home health aides on a two-week cycle.

Required MA RN license and BLS; 1 year of direct care nursing; OASIS/EPIC experience preferred. Competitive salary, generous PTO, 403(b), and tuition reimbursement are offered.

Qualifications

• Active Massachusetts RN license.
• BLS certification required.
• At least 1 year of direct care nursing experience.
• OASIS and/or EPIC experience preferred.

Responsibilities

• Complete and document an initial assessment of patient and family.
• Initiate and revise plan of care as patient status changes.
• Administer medications and treatments as prescribed.
• Instruct and evaluate the home health aide care every two weeks.
• Identify discharge planning needs as part of care planning.
• Maintain confidentiality per HIPAA guidelines and complete OASIS.
• Document through EPIC in a timely manner.
