Job summary

Beth Israel Lahey Health is seeking an experienced registered nurse to lead home health services in a community setting. The orientation training program offers hands-on hospice training with designated preceptors and ongoing mentorship.

The role emphasizes planning, directing home care and patient-family education, with a strong focus on community health and compliance with home care regulations and payer guidelines.

Qualifications

• Active New Hampshire RN license.
• Active American Heart Association BLS certification.
• Associate’s Degree or BSN from an accredited school of nursing.
• At least 1 year of direct care nursing experience, preferably home care.
• OASIS and/or EPIC experience preferred.

Responsibilities

• Complete and document an initial assessment of patient and family to determine home care needs.
• Initiate the plan of care and revise as patient status changes.
• Administer medications and treatments as prescribed by the physician.
• Instruct, supervise, and evaluate the home health aide care provided every two weeks.
• Identify discharge planning needs as part of the care plan development.
• Practice confidentiality principles set by the agency and HIPAA guidelines.
• Demonstrate proficiency in OASIS completion.
• Complete documentation through EPIC in accordance with agency timeliness policies.
