Job summary

Qualifications

• Registered nurse with license to practice in MA/NH.
• BSN strongly preferred.
• Experience with home health and OASIS is advantageous.
• Excellent observation, problem-solving and communication skills.

Responsibilities

• Coordinate patient care for home health caseload, delegating to LPN staff and monitoring outcomes.
• Conduct initial patient/family assessment and develop a care plan with goals.
• Administer medications and treatments as prescribed; provide patient and family education.
• Communicate with physicians, team members, and community providers to align care.
