Job summary

Ashtabula Regional Medical Center seeks an experienced Home Health Registered Nurse to manage a defined caseload in Ashtabula County. You will assess patients, develop and implement care plans, coordinate with physicians, and evaluate outcomes to ensure quality home-based care.

In this role, you will work independently within an on-site home health setting, using EMR systems to document assessments and interventions.

• Strong clinical and assessment skills.
• Ability to work independently.
• Strong interpersonal communication skills.
• Ability to document in an EMR.

Responsibilities

• Act as primary care manager for a caseload of Home Health patients; assess, plan, implement and evaluate the plan of care in coordination with the patient’s physician.
• Coordinate with other health care providers to ensure quality home-based care and timely follow-up.
