Homecareconcierge is seeking a Registered Nurse to provide one-on-one home health services, coordinating comprehensive care plans and delivering patient-centered nursing care in patients' homes across Ohio.

The role emphasizes independence, clinical judgment, and collaboration with an interdisciplinary team to ensure quality outcomes and strong patient relationships in a community setting.

Qualificações

• Licensed RN in the state where practicing.
• Current CPR certification.
• Valid driver’s license and reliable vehicle.

Responsabilidades

• Complete an initial assessment of patient and family to determine home care needs.
• Regularly re-evaluate patient nursing needs.
• Initiate and revise the plan of care as patient status changes.
• Develop and implement a care plan with patient/family involvement.
• Administer medications and treatments as prescribed.
• Counsel patient and family on nursing and related needs.
• Provide health education to patients and families.
• Identify discharge planning needs and coordinate prior to discharge.
