Job summary

Cleveland Clinic Home Care Services is seeking an experienced RN to provide high-quality nursing care to patients in their homes within the Cleveland Clinic system. You will perform assessments, deliver complex treatments, and educate patients to prevent readmissions while coordinating with a multidisciplinary team.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN) in Ohio.
• BLS Certification through the American Heart Association.
• Valid Ohio Driver's license and proof of auto insurance on hire.
• One year of RN experience in acute Med/Surg or Home Care.

Responsibilities

• Provide direct nursing care to patients in home settings.
• Perform admission assessments and timely reassessments.
• Assess safety risks, abuse and neglect.
• Coordinate care across disciplines for well-coordinated patient care.
• Assess, document, and manage patients' pain.
