Zusammenfassung

Ashtabula Regional Medical Center seeks an experienced Home Health Registered Nurse to manage a defined caseload in Ashtabula County. You will assess patients, develop and implement care plans, coordinate with physicians, and evaluate outcomes to ensure quality home-based care.

In this role, you will work independently within an on-site home health setting, using EMR systems to document assessments and interventions.

Required Length And Type Of Experience

• One year nursing experience preferred

Required Licensure Or Certification

Current State of Ohio Registered Nurse license

• Current Valid State Driver's License (VDL)
• Minimum of two years of driving experience; minimum of five years of driving experience if transporting patients and/or visitors.
• Zero points preferred on driver’s license and cannot exceed six points total allowable.
• Maintain evidence of personal auto insurance.
• Assume responsibility to pay fees associated with traffic violations.
• Must maintain eligibility of insurability as determined by ARMC's insurance carrier's requirement to operate organization’s vehicles.

Required Physical And Environmental Demands

Requires eye/hand coordination and manual dexterity sufficient to operate a telephone, analyze and prepare written records, and provide hands-on care to clients. Requires normal range of hearing to communicate with staff, clients, physicians, and other health care providers. Must be able to lift and carry 25 lbs.

General Summary

The Home Health RN acts as a primary care manager for a caseload of patients. The RN’s role is to assess, plan, implement, and evaluate the plan of care coordinating with the patient's physician. The case manager functions in an assigned territory within Ashtabula County.
