Job summary

Atrium Health is seeking an experienced Home Health Nurse to manage an assigned patient caseload in the Cleveland area. The role emphasizes timely care, data collection from assessments and EMR, and effective care coordination with a multidisciplinary team.

The ideal candidate will be a graduate nurse with an RN license (BSN preferred), and BLS certification. This position includes a sign-on bonus eligibility and comprehensive benefits.

Qualifications

• Graduate from an accredited School of Nursing.
• BSN preferred.
• Current RN license in the state of work (or compact state).
• BLS certification required.

Responsibilities

• Manage daily patient caseload in home health with timely care and documentation.
• Collect patient data from assessments, telephonic contacts, and EMR.
• Analyze data to determine issues and resources, mitigate risk, and ensure quality outcomes.
• Coordinate care with multidisciplinary team and supervise LPNs/HHAs as needed.
• Perform orders and treatments per policy and regulations; evaluate progress toward goals.
