Qualifications

• Graduate from an accredited school of professional nursing
• BSN preferred
• Minimum one year of current clinical experience as a registered nurse
• One year of clinical experience as an RN in an acute med/surg setting
• Current RN License in Ohio
• Basic Life Support (BLS) certification required
• Valid Ohio driver’s license and proof of automobile insurance coverage

Responsibilities

• Provide direct nursing care to patients in their homes.
• Perform admission assessments and reassessments.
• Assess patients for safety risks, abuse, and neglect.
• Serve as a primary coordinator of all disciplines for patient care.
• Assess, document, prioritize, and manage patients’ pain.
• Complete documentation and make calls after patient visits.
• Attend meetings and coordinate patient care.
• Arrange visit schedules for patients.
