TGH Home Care powered by VNA of Florida is seeking a compassionate Registered Nurse to provide in-home health care to patients in the Brandon, Valrico, and North Riverview area. The RN will coordinate, deliver, and document nursing care in patients' homes, focusing on quality outcomes and family education.

Qualifications

• Active Registered Nurse license required.
• Minimum of 1 year clinical experience.
• Ability to work independently with excellent communication.

Responsibilities

• Plans, coordinates, and ensures quality home health care for patients and families.
• Delivers professional nursing services in the home setting and provides case management.
• Performs initial assessments and monitors progress toward goals.
• Maintains clinical documentation including OASIS and care coordination.
• Collaborates with physicians and discharge planners to promote health education.
