TGH Home Care powered by VNA of Florida is seeking a Registered Nurse (RN) for home health in Sun City and surrounding areas. You will provide skilled nursing care in patients' homes, coordinate case management, and ensure quality outcomes with a patient-centered approach.

The role emphasizes flexible PRN scheduling, collaboration with the care team, and ongoing education to support patients and families in maintaining independence at home.

Qualifications

• Active Registered Nurse License required.
• Minimum 1 year clinical experience.
• Strong communication and organizational skills.
• Ability to work independently.
• Valid driver's license and reliable transportation (if applicable).
• Commitment to compassionate, patient-centered care.

Responsibilities

• Plans, coordinates, and ensures quality home health care for patients and families.
• Delivers professional nursing services in the client’s home and provides case management.
• Monitors changing client needs and progress toward goals.
• Performs initial assessments and documents care coordination (OASIS, evaluations, orders).
• Maintains communication with patient, families, physicians, and office staff.
• Collaborates with Discharge Planners and physicians; promotes health education.
• Interacts with community health resources and participates in education and development.
