Responsibilities

• Develops work methods and procedures to deliver high-quality care.
• Delivers nursing services in the client's home and provides case management.
• Monitors changing needs of the client and family.
• Provides initial assessment and tracks progress toward goals.
• Maintains communication with patient, families, physicians, and care team.
• Completes clinical documentation including OASIS and orders.
