Job summary

TGH Home Care powered by VNA of Florida is seeking a Registered Nurse (RN) for home health visits in the Tampa area, including Lutz, Odessa, West Chase, South Tampa, New Tampa, and Carrollwood. The role focuses on planning, coordinating, and delivering quality care at patients' homes with strong emphasis on assessment and care coordination.

Qualified candidates hold an active RN license and at least 1 year of clinical experience, with excellent communication and organization skills.

Qualifications

Active Registered Nurse License required.
Minimum of 1-year clinical experience.
Strong communication and organizational skills.
Ability to work independently.
Valid driver's license and reliable transportation (if applicable).

Responsibilities

Develops work methods and procedures to deliver high-quality home care.
Delivers professional nursing services in patients' homes and provides case management.
Monitors changing needs of the client and family.
Performs initial assessment and tracks goals and outcomes.
Maintains communication with patient, families, physicians, and staff.
Participates in interdisciplinary team conferences.
Implements and monitors the plan of care with patient involvement.
Completes clinical documentation (OASIS, evaluations, orders).
Collaborates with physicians and discharge planners to educate patients and caregivers.
Lorges liaison with community resources (DME, oxygen, infusions).
Interacts with patients, families, team members, and health professionals.
Pursues ongoing education and professional development.
