TGH Home Care powered by VNA of Florida in Florida seeks a Registered Nurse (RN) to deliver high-quality home health care. The RN will plan, coordinate, and provide nursing services in patients' homes, with emphasis on safety, comfort, and family education.

You will develop care plans, perform initial assessments, monitor progress, document care (OASIS related), and collaborate with physicians and discharge planners to support patients' independence.

Qualifications

• Active Registered Nurse license is required.
• Minimum 1 year of clinical experience.
• Strong communication and organizational skills.
• Ability to work independently and manage in-home care.
• Valid driver's license and reliable transportation, if applicable.

Responsabilités

• Plans, coordinates, and assures quality care in the home setting.
• Delivers professional nursing services and provides case management.
• Monitors patient needs and progress toward goals.
• Completes clinical documentation, including assessments and care coordination.
• Collaborates with physicians, discharge planners, and care teams.
• Educates patient and family and liaises with community resources.
