Job summary

TGH Home Care powered by VNA of Florida is seeking a Registered Nurse (RN) to provide in-home health services in Sun City, Wimauma and nearby areas. The RN will plan, coordinate, and ensure quality care for patients and families.

Qualifications

• Active Registered Nurse license required.
• Minimum of 1 year clinical experience.
• Strong communication and organizational skills.
• Ability to work independently.
• Valid driver's license and reliable transportation (if applicable).
• Commitment to compassionate, patient-centered care.

Responsibilities

• Develops work methods and procedures to render high-quality care.
• Delivers professional nursing services in the client’s home and provides case management.
• Monitors changing needs of the client and family.
• Provides initial assessment and monitors progress toward goals.
• Maintains communication with patient, families, physicians, and staff.
• Participates in interdisciplinary conferences and coordinates plan of care.
• Completes clinical documentation (including OASIS, evaluations, orders).
• Collaborates with physicians and discharge planners to promote health and education.
• Interacts with patients, families, team members, and other health professionals.
