Job summary

VNA of Florida is seeking a Registered Nurse (RN) for home health in the Brandon, Valrico, North Riverview area. The RN provides skilled nursing care in patients' homes, develops and implements plans of care, and coordinates services with families, physicians, and the care team.

This PRN position offers flexible scheduling, meaningful work, and opportunities for professional development as part of a long-standing home health leader serving Florida for 50 years.

Qualifications

• Active RN license in Florida required.
• Minimum 1 year of clinical experience.
• Valid driver's license and reliable transportation (if applicable).

Responsibilities

• Plans, coordinates, and ensures quality care for home patients.
• Delivers nursing services in clients' homes and manages caseload.
• Monitors patient needs and progress toward goals.
• Conducts initial assessments and tracks clinical outcomes.
• Maintains communication with patients, families, physicians, and care team.
• Participates in interdisciplinary conferences.
• Develops and monitors the care plan with patient input.
• Completes clinical documentation including OASIS and orders.
• Collaborates with physicians and discharge planners to promote health education.
• Acts as liaison with community health resources.
• Engages in ongoing education and professional development.
