Job summary

VNA of Florida is seeking a Registered Nurse (RN) for Home Health in the Boynton Beach, Delray Beach, Boca Raton, and S West Palm Beach areas. The role is full-time and focuses on delivering skilled nursing visits in patients’ homes, completing OASIS assessments, and coordinating with a multidisciplinary team to ensure optimal patient outcomes.

The ideal candidate holds an active Florida RN license with at least one year of nursing experience and preferably home health experience or OASIS

Qualifications

• Active Florida RN license is required.
• Minimum one year of nursing experience.
• Home health experience preferred.
• OASIS experience preferred.
• Strong assessment and critical thinking skills.
• Ability to work independently.

Responsibilities

• Perform skilled nursing visits in patient homes.
• Complete OASIS assessments and documentation.
• Develop and manage individualized care plans.
• Educate patients and caregivers on disease management and safety.
• Coordinate care with physicians and interdisciplinary team members.
• Monitor patient progress and communicate changes in condition.
