Job summary

Granite Peak Home Health is seeking a Registered Nurse to join our growing team in Montana. You will provide nursing care per the plan of care, perform initial evaluations, and reassess patient needs on an ongoing basis.

The role emphasizes independent judgment, collaboration with therapists, and adherence to HIPAA and agency policies. The position offers a competitive salary, mileage reimbursement, and a flexible schedule within a Monday–Friday framework with on-call as needed.

Qualifications

• Graduate of an approved school of professional nursing and currently licensed in Montana.
• Two (2) years nursing experience; 1 year home health preferred.
• Must hold a BLS/CPR certificate.
• Ability to exercise initiative and independent judgment.
• Familiarity with HIPAA compliance and patient privacy requirements.
• Knowledge of local healthcare market and community outreach strategies.
• Knowledge of WellSky (EHR) and basic MS Office tools.

Responsibilities

• Provides services by the plan of care.
• Makes the initial evaluation visit and reevaluates patient nursing needs.
• Initiates the plan of care and revisions as needed.
• Provides services requiring specialized nursing skills and preventive procedures.
• Prepares notes and summaries per agency policy.
• Coordinates services and informs staff of patient condition changes.
• Counsels patients and families on nursing needs and care plans.
• Supports peer review and quality assurance activities.
