Apollo Home Healthcare, part of Obran Cooperative, is hiring an RN to join our Home Health team serving the Bay Area. You will provide high-quality in-home nursing, oversee a LVN, and collaborate with physicians, therapists, and a clinical team to coordinate patient care.

The role offers flexible scheduling for full-time or part-time/per diem clinicians, with care delivered across Alameda, Contra Costa, and Solano counties.

Qualificações

• Active California RN license required.
• CPR/BLS certification required.
• Strong communication and organizational skills are essential.
• Reliable transportation and willingness to travel to patients\' homes.
• Comfortable working independently in a home health setting; travel across counties.

Responsabilidades

• Provide in-home nursing visits according to the physician\'s plan of care.
• Assess patients\' health status, history, and home environment.
• Develop and update individualized care plans.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on disease management and home care.
• Monitor patient progress and communicate changes to the physician and care team.
• Document visits and care in a timely manner.
• Coordinate care with therapists, aides, and other team members.
• Serve as case manager for assigned patients and support discharge planning.
