Apollo Home Healthcare is hiring a Registered Nurse (RN) to join our Home Health team in Northern California. The role supports patients in their homes, coordinating with physicians, therapists, and a clinical team to deliver high-quality care.

Qualifications

• Active California RN license is required.
• CPR/BLS certification is required.
• Strong communication and organizational skills are essential.
• Reliable transportation and willingness to travel to patients' homes.
• Comfortable working independently in a home health setting.
• Home health experience preferred, software experience is a plus.

Responsibilities

• Provide in-home nursing visits per physician's plan of care.
• Assess health status, history, and home environment.
• Develop and update individualized care plans.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on disease management and home care.
• Monitor progress and report changes to care team and physician.
• Document visits and care in a timely manner.
• Coordinate care with therapists, aides, and other team members.
• Serve as case manager and support discharge planning.
