Job summary
Allina Health in Minneapolis, MN seeks an experienced Home Care Registered Nurse to provide comprehensive patient assessments and individualized care plans. You will collaborate with families and the healthcare team to ensure safe, high-quality home-based care and ongoing patient support.
Flexibility and weekend coverage are required; mileage reimbursement and per-visit pay are provided. A valid MN RN license and BLS are required, with ongoing orientation and training to maintain competency.
Qualifications
- Associate's or Vocational degree in Nursing is required.
- 2 to 5 years homecare/hospice RN experience preferred.
Responsibilities
- Principle Responsibilities: comprehensive assessment of patient and family needs; establish plan of care; supervise progress and modify plan as needed.
- Maintain education to remain proficient in high-quality patient care practice.
- Coordinate with family, patient, and other disciplines to ensure safe, effective care delivery.
- Demonstrate ability to coordinate, delegate, and direct unit operation when acting as Charge Nurse or Preceptor.
- Other duties as assigned.
