Qualifications

  • Minimum 1 year medical/surgical or home health experience preferred.
  • WellSky (Kinnser) experience preferred.
  • Current TB test within 12 months.
  • Current CPR Certification.
  • Valid driver’s license and proof of liability insurance on automobile.
  • Ability to attend 3-day orientation at our Minnetonka office.
  • Ability to work 8 AM – 5 PM Monday – Friday with one weekend a month on call.

Responsibilities

RNs Needed!

Nightingale Home Healthcare is expanding rapidly and seeking full‑time Registered Nurse Case Managers to fulfill our growing census. Nightingale RNs administer skilled nursing care to patients requiring intermittent professional nursing service and teaches and/or supervises the family and other members of the nursing team. These services are performed in accordance with the physician’s orders and under the direction and supervision of the Director of Clinical Services.

This is a PAY PER VISIT position with the ability to make 120k yearly!! Routine visits starting at $60 per visit!

Duties Include
  • Obtains medical history, particularly as it relates to the present condition, from the patient and/or family member.
  • Conducts a brief physical examination of the patient, including vital signs, ambulatory status, mental status, bowel and urinary regularity, appetite and type of diet.
  • Evaluates the patient, family member(s) and home situation to determine what health teaching will be required.
  • Evaluates the patient’s condition to determine what assistance will be available from family members caring for the patient.
  • Evaluates the patient’s condition and home situation, determine if the services of a LPN, Home Health Aide, Personal Care Attendant will be required and the frequency of this situation.
  • Interprets nursing and other services of the agency to patients and families as part of planning of care.
  • Develops and implements the nursing care plans.
  • Provides skilled nursing care as outlined in the nursing care plan to include, but not be limited to the following:
    • Nursing services, treatments, and preventative procedures requiring substantial specialized skill and ordered by a physician.
    • Initiating preventative and rehabilitative nursing procedures as appropriate for the patient’s care and safety.
    • Observing signs and symptoms and reporting to the physician reactions to treatments, including drugs and changes in the patient’s physical or emotional conditions.
    • Teaching, supervising and counseling the patient and family members regarding the nursing care needs and other related problems of the patient at home.
  • Coordinates and supervises the care given by the LPN, Home Health Aide and/or Personal Care Attendant assigned to provide care to the patient.
  • To follow Physician orders, maintain Plan of Care that is accurate and current for each patient based on comprehensive assessments, MD orders and is reflective of patient’s needs.
  • Charts those services the Visiting Nurse – RN has rendered to the patient, notes changes in the patient’ condition and/or family and home situation, makes revisions in the nursing care plan as needed, records supervisory visits, evaluates patient care and progress and closes charts of discharged patients by preparing Discharge Summaries and obtaining Discharge orders.
  • Maintains confidentiality of client’s situation, records and services provided according to HIPAA regulations
  • Evaluates the effectiveness of his/her nursing service to the individual and family.
  • Consults with the attending physician concerning alterations of the plan of treatment under direct supervision of the Director of Clinical Services.
  • Submits all patient notes and/or visit records on a timely basis.
  • Discusses with the Director of Clinical Services problems concerning the patients and how these may best be resolved as well as the patient needs and how these can be best met
  • Obtains orders for paramedical services and submits referral to the Director of Clinical Services, i.e., physical therapy, medical social services, etc.
  • Coordinates total patient care.
  • Cooperates with other agencies providing nursing or related services to provide continuation of care and to implement a comprehensive care plan.
  • Continually strives to improve nursing care by broadening and deepening knowledge through formal education, attendance at workshops, conferences and active participation in professional and related organizations and reading.
  • Participates in interdisciplinary team conferences.
Qualifications
  • Licensed Registered Nurse in MN.
  • Minimum of 1 year medical/surgical environment, Home Health (OASIS) or geriatric nursing experience preferred.
  • Wellsky (Kinnser) experience preferred.
  • Current TB results (within last 12 months).
  • Current CPR Certification.
  • Valid driver’s license and proof of liability insurance on automobile.
  • Ability to attend 3-day orientation at our Minnetonka office.
