BrightStar Care of Baraboo is hiring registered nurses to provide one-on-one care to clients in Baraboo and surrounding communities. As a Home Care Nurse you will deliver personalized nursing care and coordinate clinical services in clients' homes, supporting health, safety, comfort, and independence.
We offer flexible scheduling, competitive hourly pay based on experience, and the option for full-time or part-time hours.
Formación
- Current unrestricted Wisconsin RN license.
- Graduate of an accredited nursing program.
- Minimum one year of clinical nursing experience.
- Valid driver's license with reliable transportation.
- Strong communication, organizational, and assessment skills.
- Ability to work independently and collaborate with a multidisciplinary team.
- Proficiency with computers and electronic documentation systems.
Responsabilidades
- Perform nursing assessments and identify changes in client conditions.
- Develop or update individualized plans of care.
- Provide skilled nursing care, treatments, and procedures within scope of practice.
- Monitor client outcomes and communicate significant changes.
- Maintain accurate clinical documentation.
- Coordinate with providers, clients, and families as appropriate.
- Educate clients and families about the plan of care.
- Uphold infection prevention, safety, and emergency procedures.
