Job summary

BrightStar Care is seeking a compassionate, skilled Home Health Registered Nurse (RN) to deliver high-quality care to patients in their homes. You will conduct comprehensive assessments, Start of Care visits, care coordination, wound care management, and perform lab draws.

This role offers autonomy, building relationships with patients and families, and the opportunity to help patients heal and maintain independence in the comfort of their own homes.

Qualifications

• Current Texas Registered Nurse (RN) license in good standing.
• Current CPR/BLS certification.
• Minimum one year of nursing experience.
• Strong assessment, organization, and communication skills.
• Valid driver's license, reliable transportation, and current auto insurance.
• Knowledge of home health guidelines is a plus.

Responsibilities

• Complete SOC, Recertification, Resumption of Care, Evaluation and Discharge visits.
• Perform comprehensive nursing assessments and develop individualized plans of care.
• Monitor patient progress and communicate changes to the physician and care team.
• Coordinate home health services with physicians, therapists, and caregivers.
• Administer medications and monitor treatment effectiveness.
• Educate patients and caregivers on disease processes, medications, wound care, and safety.
