BrightStar Care of Baraboo is hiring registered nurses to provide one-on-one care to clients in Baraboo and surrounding communities. As a Home Care Nurse you will deliver personalized nursing care and coordinate clinical services in clients' homes, supporting health, safety, comfort, and independence.

We offer flexible scheduling, competitive hourly pay based on experience, and the option for full-time or part-time hours.

Formación

• Current unrestricted Wisconsin RN license.
• Graduate of an accredited nursing program.
• Minimum one year of clinical nursing experience.
• Valid driver's license with reliable transportation.
• Strong communication, organizational, and assessment skills.
• Ability to work independently and collaborate with a multidisciplinary team.
• Proficiency with computers and electronic documentation systems.

Responsabilidades

• Perform nursing assessments and identify changes in client conditions.
• Develop or update individualized plans of care.
• Provide skilled nursing care, treatments, and procedures within scope of practice.
• Monitor client outcomes and communicate significant changes.
• Maintain accurate clinical documentation.
• Coordinate with providers, clients, and families as appropriate.
• Educate clients and families about the plan of care.
• Uphold infection prevention, safety, and emergency procedures.

What You'll Do

Responsibilities may vary based on each client's needs, the individualized plan of care, the position, and the applicable scope of practice and may include:

• Completing nursing assessments and identifying changes in a client's condition
• Developing, reviewing, or updating individualized plans of care, as assigned and permitted
• Providing skilled nursing care, treatments, and procedures within the applicable scope of practice
• Monitoring client outcomes and promptly communicating significant changes or concerns
• Maintaining accurate, detailed, and timely clinical documentation
• Coordinating with providers, the clinical team, clients, and family members as appropriate
• Providing client and family education related to the plan of care
• Providing clinical supervision or oversight when included in the position's responsibilities
• Following infection-prevention, safety, and emergency procedures
• Maintaining client dignity, privacy, and confidentiality

What We're Looking For

Required Qualifications:

• Current unrestricted Wisconsin Registered Nurse (RN) license
• Graduate of an accredited school of nursing
• Minimum one year of clinical nursing experience
• Valid driver's license, reliable transportation, and automobile insurance
• Strong communication, organizational, and clinical assessment skills
• Ability to work independently, exercise sound clinical judgment, and collaborate effectively with a multidisciplinary team
• Proficiency with computers and electronic documentation systems

Preferred Qualifications:

• Home care, home health, hospice, or community-based nursing experience
• Experience performing client assessments and care planning
• Experience providing patient and family education
