Job summary

BrightStar Care is seeking a compassionate Home Health Registered Nurse (RN) to deliver high-quality care to patients in their homes. The RN will perform SOC visits, comprehensive assessments, wound care, lab draws, and education for patients and caregivers.

Ideal candidates enjoy autonomous work, building relationships with families, and coordinating care with a multidisciplinary team to achieve optimal patient outcomes in the home setting.

Qualifications

• Current Texas Registered Nurse license in good standing.
• Current CPR/BLS certification.
• Minimum one year of nursing experience.
• Strong assessment, organization, and communication skills.
• Valid driver's license, reliable transportation, and current auto insurance.

Responsibilities

• Complete Start of Care (SOC), Recertification, Resumption of Care, Evaluation, and Discharge visits.
• Perform comprehensive nursing assessments and develop individualized plans of care.
• Monitor patient progress and communicate changes in condition to the physician and care team.
• Identify patient needs and coordinate appropriate home health services.
• Coordinate care with physicians, therapists, patients, caregivers, and community resources.
• Collaborate with interdisciplinary team members to ensure continuity of care.
