Job summary

BrightStar Care is seeking a compassionate Home Health Registered Nurse (RN) to deliver high-quality care to patients in their homes. You will perform SOC visits, comprehensive assessments, wound care, and education for patients and caregivers.

The role emphasizes autonomy, relationship-building with families, and collaboration with an interdisciplinary team to ensure optimal outcomes in the home setting.

Qualifications

• Current Texas RN license in good standing.
• Current CPR/BLS certification.
• Minimum one year of nursing experience.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform Start of Care (SOC), Recertification, Resumption of Care, Evaluation, and Discharge visits.
• Conduct comprehensive nursing assessments and develop individualized plans of care.
• Monitor patient progress and communicate changes in condition to the physician and care team.
• Coordinate care with physicians, therapists, patients, caregivers, and community resources.
• Collect laboratory specimens and perform lab draws as ordered.
