Job summary

CVHCare in California is seeking a Weekend Home Health RN (Field) to join an established home health team. This part-time role covers 18 visits per week and requires on-site training in San Ramon.

You will monitor patients, develop plans of care with the primary physician, act as case manager, and provide education on disease management, wound therapy, post-surgical care, and ostomy care. CA RN license and CPR/BLS are required.

Qualifications

• Minimum 1 year RN experience, preferably in acute, SNF, Home Health or Hospice setting.
• Active CA Registered Nursing License is required.
• AA, BSN, MSN degrees preferred.
• Current CPR or BLS certification is required.

Responsibilities

• Monitor patient health and coordinate with the primary care physician to develop an individualized plan of care.
• Function as a case manager delivering quality home health nursing services.
• Provide patient education on disease management and care plans.
• Deliver wound therapy, post-surgical care management, and ostomy care management as appropriate.
