Job summary

CVHCare, a leader in Home Health Clinical services, is seeking a Registered Nurse for field-based work in San Ramon, CA. The RN will monitor patient health, develop care plans with physicians, and travel to patients’ homes to deliver skilled home health services.

The role requires a CA RN license and at least 1 year of RN experience, preferably in home health or related settings. Training 5 days onsite is provided, with benefits including medical, dental, vision, 401K, life insurance, holiday

• Minimum 1 year RN experience in acute, SNF, Home Health or Hospice settings preferred.
• Willingness to train for 5 days onsite in San Ramon, CA.

Responsibilities

• Monitor patient health and develop individualized care plans with the primary care physician.
• Act as a case manager delivering quality home health care.
• Educate patients on disease management and care plans.
• Provide wound therapy, post-surgical care, and ostomy care as needed.
• Schedule and visit patients in their homes, maintaining independent planning.

Title: Home Health RN (Field)

Schedule: Full Time (24 RVE p/week) or Part Time (18 RVE p/week) must be available to work up two (2) weekends per month

• Paid Training: Must be able to train for 5 days Mon-Friday 9am-5:30pm in San Ramon (onsite)

Position Summary: The Home Health RN monitors a patient’s overall health, and in partnership with a primary care physician, develops an individualized plan of care for their patients. Each RN functions as a case manager, and works independently to deliver quality healthcare to their patients. As well as customized care and patient education about disease management, the Home Health RNs provide a variety of specialized services, such as Simple to Complex Wound Therapy, Post-Surgical Care Management, and Ostomy Care Management. truly allowing you to function independently, with the flexibility to make your own schedule and plan patient visits around other commitments if needed. The home care setting allows you quality time with your patient in the comfort of their own home.

Minimum 1 year RN experience preferably in acute, SNF, Home Health or Hospice setting

Must Possess an active CA Registered Nursing License

Compensation is based on full time schedule
