Qualifications

• Active MA RN license.
• BLS certification required.
• 1+ year of direct patient care in home health/public health preferred.
• OASIS and/or EPIC experience preferred.

Responsibilities

• Plan, organize, and direct home care services.
• Initiate and revise the plan of care as patient needs change.
• Administer medications and treatments per physician orders.
• Instruct and supervise home health aide care every two weeks.
• Identify discharge planning needs as part of care plan development.
• Maintain HIPAA confidentiality in all patient interactions.
• Complete OASIS documentation and EPIC timely.
