Job summary

Home Care Network, Inc. seeks an RN Case Manager/Recertification Nurse (RNCM) for Northwest Ohio. This Part-Time/PRN leadership role involves travel to multiple counties, with a focus on SOC, admissions, recertifications, and supervisory reviews.

Qualifications

• Minimum 1 year in case management, admissions/discharges, wound care, or transitional care.
• Experience with home health agency or in-home care is preferred.
• Strong documentation and supervisory abilities.

Responsibilities

• Coordinate, re-evaluate and confirm medical necessity of continued home health care services.
• Oversee patient services, including assessments, discharge planning, and care plans.
• Lead a team of RN/LPN/HHA with emphasis on compliance and quality.
