Home Care Network, Inc. in Northwest Ohio seeks a part-time or PRN RN Case Manager (RNCM) / Recertification Nurse to lead field care teams across Findlay, Fostoria, Tiffin and surrounding counties.

The role focuses on case management, SOCs, admissions, recertifications, transitions and ROC, with travel required and no overnight shifts. You will mentor staff, ensure high-quality care, and support training and outcomes across the agency.

Qualifications

• At least 1 year of experience in Case Management, Admissions/Discharges, wound care, PICC/PORT/IV care, transitional care, lab draws, OASIS and non-OASIS assessments, and strong documentation/leadership.

Responsibilities

• Manage case load including SOCs, admissions, recertifications and ROC; conduct clinical reviews and support training and care practices.
