Mission Healthcare in California seeks a qualified Registered Nurse to provide home health and hospice care. You will perform initial patient assessments, develop and adjust care plans, and deliver comprehensive nursing services in patients’ homes.

The role requires an active RN license, BLS/CPR, and at least 1 year of acute care with home health experience preferred. Schedule includes full-time with weekend/on-call rotations, in Poway/Ramona/Rancho Penasquitos/Rancho Bernardo area.

Qualifications

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• BLS, with hands-on CPR certification.
• Valid driver’s license and auto insurance.
• Associate’s or Bachelor’s degree in Nursing, and 1 year of home health experience preferred.

Responsabilités

• Completes an initial assessment of the patient and family to determine home care needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Regularly re-evaluates patients’ needs.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan which establishes goals based on nursing diagnosis, and involves patient and family.
• Initiates preventive and rehabilitative nursing procedures; administers medications as prescribed.
• Counsels patient and family in meeting plan of care and related needs.
• Provides health care instructions appropriate to assessment and plan.
• Identifies discharge planning needs and implements prior to discharge.
