Qualifications

Responsibilities

• Completes initial patient and family assessment to determine home care needs.
• Regularly re-evaluates patient nursing needs.
• Initiates and revises the plan of care as patient status changes.
• Uses health data to determine nursing diagnoses and goals.
• Develops a care plan with patient/family involvement.
