Job summary

ABC Home Health Care LLC in San Diego, CA seeks a Registered Nurse to plan, organize and direct home care services, drawing on community health experience to meet patient needs at home and in the community.

You will perform initial assessments, develop and revise care plans, administer medications, and educate patients and families. This role requires California RN licensure, CPR, and at least 2 years of licensed nursing experience in California.

Qualifications

• Graduate of an accredited school of nursing.
• Minimum of 2 years licensed nursing experience in California.
• Current California RN license and CPR certification.
• Bachelor’s degree with 1 year home health care experience preferred.
• Management experience not required. Responsible for supervising home health aides.
• Excellent observation, verbal and written communication skills; problem solving; basic math skills.

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs.
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and revises as patient status changes.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan with goals based on nursing diagnosis and includes patient and family.
• Administers medications and treatments as prescribed by the physician.
• Counsels the patient and family in meeting nursing needs.
• Provides health care instructions to patient per plan of care.
• Identifies discharge planning needs and implements prior to discharge.
• Acts as Case Manager and coordinates patient care for assigned caseload.
