Job summary

Bridge Home Health & Hospice is seeking aRegistered Nurse to plan, organize and direct home care services. You will conduct comprehensive patient assessments, develop and revise care plans, and administer medications in patients' homes with emphasis on family involvement and community health education.

The role requires California licensure, CPR, and strong communication skills. You will collaborate with physicians, supervise aides, and participate in on-call duties to ensure quality patient

Qualifications

• Graduate of an accredited school of nursing.
• Minimum of 2 years licensed experience (California).
• Current licensure in state and CPR certification.
• Bachelor’s degree, with one (1) year of home health care experience preferred.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills.
• Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment.
• Must possess a valid California Driver’s License with automobile in good working order.

Responsibilities

• Completes initial patient and family assessment to determine home care needs.
• Regularly re-evaluates patient nursing needs and revises the plan of care accordingly.
• Initiates the plan of care and adjusts as patient status changes.
• Utilizes health data to determine nursing diagnoses and develops goal-oriented care plans.
• Administers medications and treatments as prescribed; provides direct patient care.
• Counsels patient and family on nursing needs and care plans.
• Communicates with primary physician and coordinates with community health resources.
• Participates in on-call duties and ensures equipment and supplies are available.
• Trains/supervises home health aides and supports reliability guidelines.
