AccentCare, Inc. is seeking an experienced RN Case Manager to oversee patient care plans in Lebanon County, PA and surrounding areas. The role combines direct and indirect patient care, coordinating RNs, LPNs and CHHAs to deliver individualized plans.

The position requires PA RN licensure, at least 1 year of RN experience and CPR certification, with the opportunity to lead interdisciplinary teams and ensure quality outcomes for home health patients.

Formación

• Licensed registered nurse in the state of operation (PA).
• Minimum of one year RN experience.
• Current CPR certification.

Responsabilidades

• Assess nursing needs and manage the patient’s care plan.
• Complete initial patient assessment upon admission with history.
• Select nursing diagnoses and coordinate care with the physician.
• Review medications for adverse effects and interactions.
• Establish measurable goals aligned with diagnosis.
• Inform physician of assessment results and interventions.
• Implement nursing interventions within the Nurse Practice Act.
• Re-evaluate patient needs as required.
• Document assessment, progress and discharge notes timely.
