AccentCare in Exton, PA is seeking an RN Case Manager in Home Health to manage patient care plans from admission through discharge, coordinating with staff nurses, LPNs, and CHHAs to ensure quality care in Chester and Delaware Counties.

This role combines direct and indirect patient care. Requires PA RN license, CPR, and at least one year of RN experience; benefits include medical/dental/vision, PTO, 401(k), and wellness programs.

Qualifications

• Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of operation.
• A minimum of one year of experience as an RN.
• Current CPR Certification required.

Responsabilités

• Assess the status of nursing needs and manage the patient’s care plan.
• Complete initial patient assessment upon admission and collect history.
• Select appropriate nursing diagnoses and monitor therapies for effectiveness.
• Assess medications for safety, interactions, and allergies; identify adverse effects.
• Establish measurable goals aligned with diagnosis; communicate with physician.
