AccentCare in Chattanooga, TN is hiring an RN Case Manager for full-time home health coverage in Hamilton County. The role combines direct and indirect patient care, guiding LPNs, CHHAs, and staff nurses to deliver the plan of care from admission through discharge.

Qualified candidates are graduate from an approved school of nursing, hold an RN license in TN, have CPR, and at least one year of RN experience.

Qualifications

• RN license in the state of operation (Tennessee)
• A minimum of one year of RN experience
• Current CPR certification required

Responsabilités

• Assess nursing needs and manage patient care plans from admission to discharge
• Perform initial patient assessment and gather patient history
• Identify nursing diagnoses and coordinate interventions with physicians
• Monitor medications for effectiveness, adverse effects, and allergies
• Document assessments, progress, and discharge notes in a timely manner
• Communicate patient status changes to physicians and staff
