Job summary

Ashealthnet is seeking a Registered Nurse Case Manager to lead assessment, planning and coordination of care for patients in the Arlington, Alexandria and surrounding Virginia areas.

Qualifications

• Minimum of one year of RN experience in an acute care setting.
• Home health experience is desired.
• Ability to communicate effectively with patients, families, physicians and staff.
• Detail oriented with strong nursing judgement and timely documentation.

Responsibilities

• Oversees patient care from start to finish of episode.
• Update doctor orders in patient chart as needed.
• Notify physician of significant changes in patient condition.
• Coordinate multi-disciplinary care and supervise nursing staff.
• Administer medications as prescribed and ensure safety.
