Ashealthnet is seeking an experienced Registered Nurse Case Manager to lead assessment and planning for patient care in home health settings across Arlington, Alexandria, Falls Church and surrounding areas. You will coordinate multidisciplinary teams, educate patients and families, and ensure compliant documentation.

• Active RN license in the state of employment.
• Minimum of one year of RN experience in an acute care setting; home care preferred.
