Alternate Solutions Health Network is seeking an Registered Nurse Case Manager to lead patient assessments, care planning, and coordination with physicians and care teams in the West Chester area. The RN will educate patients and families, ensure safe care delivery, and maintain compliant documentation under Medicare guidelines.

Qualifications

• Minimum of one year of RN experience in acute care.
• Strong communication with patients, families, physicians, and staff.
• Calm, patient, compassionate, and team-oriented.
• Detail-oriented with strong nursing judgment.
• Valid RN license in the state of employment.

Responsabilités

• Oversees the overall care of the patient from beginning to end of episode.
• Updates doctor orders in patient chart accordingly.
• Notifies doctor of any significant changes in patient condition.
• Develops and adjusts an individualized plan of care using OASIS assessments.
• Administers medications as prescribed by the physician.
• Documents all clinical information in accordance with agency protocol and Medicare guidelines.
• Drives to patient locations and conducts visits as scheduled.
• Supervises care provided by nurse aides and LPNs.
• Participates in on-call requirements and case conferences.
