CareBuilders at Home – Arlington VA is seeking a PRN Registered Nurse (RN) Case Manager to provide comprehensive case management for clients at home. You will assess needs, develop care plans, and coordinate services to ensure high-quality, compassionate care.

Responsibilities

• Complete new client admissions and initial assessments.
• Develop individualized care plans based on client needs and preferences.
• Oversee adherence to care plans and visit client homes to assess care quality.
• Coordinate with physicians, therapists and social workers to ensure continuity of services.
• Maintain accurate documentation of client care and progress.
• Educate clients and families on health management and safety practices.
• Participate in case conferences to discuss progress and improvements.
• Act as liaison between clients, caregivers and healthcare professionals.
