The role requires an RN with CT license, 3+ years’ clinical experience and 2+ years in case management or discharge planning. Travel up to 55–75% around Yale New Haven area is expected.

Qualifications

• Active and unrestricted CT license.
• Minimum 3+ years clinical practical experience (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac) with Medicare members.
• Minimum 2+ years CM, discharge planning and/or home health care coordination experience.
• Registered Nurse with active state license in good standing in the region where job duties are performed.

Responsibilities

• Coordinate comprehensive case management activities for individuals and families.
• Assess needs, plan care, and facilitate access to benefits and services.
• Collaborate with providers, employers, and insurance carriers to optimize outcomes.
• Document case work activities and communicate progress to stakeholders.
